Brief Description

The Level 2—Sleep Disturbance—Adult (PROMIS Sleep Disturbance Short Form) is a brief self-report questionnaire designed to assess the severity of sleep disturbance in adults over the past 7 days. Adapted from the Patient-Reported Outcomes Measurement Information System (PROMIS), this measure evaluates key aspects of sleep quality—including difficulty falling or staying asleep, perceived sleep quality, and satisfaction with sleep. Respondents rate each of the 8 items on a 5-point Likert scale reflecting frequency or intensity of sleep-related problems. This measure is part of the DSM-5 Level 2 Cross-Cutting Symptom Measures collection, intended for ongoing symptom monitoring in clinical and research settings rather than for diagnostic purposes. It can be used to track treatment response or to complement assessments for conditions where sleep disturbance is a core or comorbid feature (e.g., depression, anxiety, PTSD, or chronic pain).

Summary of Scoring and Interpretations

The DSM-5 Level 2 Sleep Disturbance contains 8 questions scored on a 5-point Likert scale from 1-5. Items 2, 3, 7, and 8 are reverse-scored. The measure is scored by summing the scores from each of the 8 items, with total scores ranging from 8 to 40. Higher scores indicate more severe sleep disturbances. Consistently high scores may signal areas requiring further assessment or follow-up care, with clinical judgment playing a key role in interpreting the results. Total scores range from 8 to 40. Higher scores are associated with greater severity of sleep disturbance. See below for interpretation:

  • It is part of the DSM-5 Level 2 Cross-Cutting Symptom Measures, designed to monitor symptom severity and track changes over time rather than to diagnose sleep disorders.

  • Useful across a range of clinical settings, including primary care, behavioral health, pain management, and psychiatry, where sleep problems commonly co-occur with mood and anxiety disorders.
