Brief Description
The Advanced Warning of Relapse Questionnaire (AWARE 3.0) is a clinical tool designed to identify early warning signs and symptoms that may indicate a potential relapse in individuals with alcohol use disorders. The AWARE 3.0 has 28 self-report items where individuals assess frequency of thoughts, feelings, and behaviors that signal a risk of relapse. By identifying these early warning signs, clinicians can intervene proactively to prevent full-blown relapse, helping to maintain stability and improve long-term outcomes for the individual.

Summary of Scoring and Interpretations
Items are answered on a 7 point Likert scale. Sum the responses of all items, reverse scoring items 8, 14, 20, 24, and 26. Total scores can range from 28-196, with higher scores reflecting a higher probability of the respondent engaging in heavy drinking during the next two months. The probabilities for relapse or heavy drinking are represented in the following table:
